Key reposnsibilities

  • Conduct thorough patient assessments, including medical history, physical exams, and diagnostic tests.
  • Diagnose and treat acute and chronic illnesses.
  • Prescribe medications and therapies as appropriate.
  • Develop and implement individualized care plans.
  • Educate patients and their families on disease prevention, health maintenance, and management of chronic conditions.
  • Collaborate with healthcare team members to provide coordinated and comprehensive care.
  • Maintain accurate and up-to-date patient records using electronic medical record (EMR) systems.
  • Participate in ongoing professional development and continuing education activities.

Qualifications

  •  Registered Nurse Practitioner and in good standing with respective regulating body with a minimum of 3 years of clinical experience in a healthcare setting.
  • Experience in program development and education is considered an asset.
  • Strong clinical skills and knowledge of evidence based practices.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in EMR software and other relevant medical technologies.
  • Commitment to providing compassionate and patient-centered care.
  • Ability to provide services in English and in French is an asset.
