Job Summary

To support the Consultant in Rehabilitation Medicine in the WHSCT. To work effectively alongside nursing staff and members of the Multi-Disciplinary Team within the department.  To assist in the provision of medical care for patients from the acute hospital settings requiring inpatient rehabilitation following an acquired neurological disability.  The post holder will be mentored by the Consultant in Rehabilitation Medicine and will learn the skills to work autonomously when required e.g. in the service provision for therapeutic Short Stay patients with neuro disability. To work autonomously for the elective admission, inpatient care and discharge of Learning Disability therapeutic short stay clients.

 

Key Duties / Responsibilities

  • The post holder will be expected to take charge of the day to day running of the ward working with nursing staff and support / Mentorship from the Consultant when necessary.
  • The post holder will be expected to participate in Consultant ward rounds.
  • The post holder should be able to autonomously admit patients, all admissions are planned elective admissions.
  • The post holder should be competent in performing a neurological examination of a new patient.
  • The post holder should be confident working autonomously on the ward when the Consultant is absent.
  • The post holder must make clear documentation in the patients records after every consultation and on the Consultant ward round indicate at the top of the notes that the patient was reviewed by the Consultant.
  • They will be expected to accurately maintain clear readable medical chart entries and ensure medical drug Kardex are up to date at all times and ensuring that insulins/warfarin prescribing are written up for the weekend etc. (to avoid legacy work for the out of hours acute team in AAH)
  • The post holder will attend relevant MDT meetings and a weekly admission meeting when possible.  The latter will inform him/her of pending admissions over the ensuing weeks.
  • Intuitively or as directed by the Consultant, the post holder should organise specific bloods tests, radiological investigations or referral to appropriate specialists when indicated. Follow up test results, acting accordingly should anomalies arise.  The consultant should be informed if referrals are made to other specialists and copied into any correspondence.  The post holder should also inform the Consultant of any concerns.
  • The post holder should be able to perform a rapid assessment of a deteriorating patient, instigate a management plan and seek help from the Consultant or acute medical team in the hospital when necessary.
