Job Summary

The purpose of this role is to assist the implementation of phase 3 of the Respiratory Pathway across the Western Trust, in line with the aims outlined in Transforming Your Care (TYC).

The post holder will liaise closely with key stakeholders across the primary and secondary care, TYC team and Integrated Care Partnerships (ICP’s).

The post holder will assist in developing and implementing an innovative and specialist outpatients nursing service caring for patients with a range of respiratory conditions (primarily obstructive sleep apnoea) with the aim of facilitating improvements in the quality of respiratory services to patients in collaboration with members of the multidisciplinary teams in both primary and secondary care.

The post holder will run clinics in line with new pathway.

Main Duties & Responsibilities

  • Ensure that the service to be implemented supports patients with chronic Respiratory disease and incorporates the recommendations from the ICP Multi-Disciplinary Group (MDG).
  • Ensure the processes are in place that will allow for mapping of the current outpatients pathway and collate data that reflects the service as it develops.
  • Undertake clinical duties commensurate with the post.
  • Engage with key stakeholders to ensure robust policies, protocols and referral systems are in place that supports the pathway.
  • Ensure that the plan for the respiratory outpatient’s pathway is running to time and scale across all areas in a harmonised manner and all variances are brought to the attention of the Respiratory pathway project team.
  • Identify risks within the pathway, escalating these appropriately.
  • Provide leadership that ensures the ethos of the respiratory outpatients pathway is embedded within the overall integrated model of Health and Social Care.
  • Ensure effective systems of communication exist with the range of health and social care professionals who refer to the service to ensure the smooth transition and safe delivery of treatment and support through all care settings.
  • Engage with Trust ICT to ensure the most recent developments in the service have been incorporated and are user friendly to support the presentation of results.
  • Ensure that services are delivered to the standard and quality expected by the Department of Health and Social Services, Regional Authority, RQIA, and by the Trust Board.
  • Liaise with colleagues in Trust Information Department to establish baseline current service and develop systems for collating and reporting data as the service develops.
  • Assist with the development and implementation of a project plan, in line with the overall pathway plan.
