Job summary

The post holder will manage the Occupational Therapy Team within the Rehabilitation/Reablement service, assisting with the management, development and delivery of a consistently high quality service.

In addition, the post-holder will provide a high quality evidence based Occupational Therapy Service to a specialist caseload of patients with complex needs and their families and carers who require Reablement/Rehabilitation. The post-holder will provide specialist Occupational Therapy input to planning, development and evaluation of clinical services within their area of expertise, taking responsibility for defined projects. The post-holder will liaise and consult with the Homecare Reablement Team lead to plan, implement, monitor, evaluate and improve reablement service delivery.

The post holder will have operational and supervisory responsibilities for Clinical Occupational Therapy staff and students within this area of service.

Main responsibilities

  • To work autonomously to undertake the specialist comprehensive assessment and treatment of clients receiving Reablement/Rehabilitation service. These clients will have a wide range of complex needs and multi-pathologies where a diagnosis may not be formalized. Demonstrate highly specialized skills, advanced clinical reasoning and highly developed analytical skills to determine diagnosis and prognosis of condition. This will require a specialist assessment of all aspects of occupational performance (physical, cognitive, mental, social) and use this information and experiential knowledge to recommend the most appropriate Reablement/ Rehabilitate treatment plan.
  • To play a major and specialist role in the advanced assessment, treatment and clinical management of a highly complex caseload of clients who are diagnosed with acute and chronic presentations, determining the clinical diagnosis, functional deficits and occupational therapy reablement/rehabilitation treatment programme required.
  • To demonstrate knowledge of a wide range of occupational therapy assessments – both standardised and non-standardised. To use advanced clinical reasoning skills to select and assist senior staff in carrying out the most appropriate assessment for highly complex clients where current available assessment tools are inappropriate.
  • To be professional lead in screening and allocation of service referrals for both Reablement and Home Based OT Rehabilitation Services. Use advanced clinical reasoning, demonstrating critical thinking, reflection and analysis to develop appropriately graded individual and group goals/objectives for a complex caseload. Demonstrate advanced skills in activity analysis and synthesis.
  • To develop collaborative treatment plans with clients, their families, carers and Reablement home care services, demonstrating expert clinical reasoning and problem solving techniques to determine agreed outcomes/goals. To monitor and review intervention plans in conjunction with reablement home care staff.
  • To have complete clinical autonomy to determine the management of clients with specific conditions in your area of expertise, seeking advice when appropriate, from expert members of the multidisciplinary team eg consultant.
  • To be professionally and legally accountable for all aspects of own work, including the management of clients in your care and monitor the work of others within the team.
  • To act as a source of occupational therapy specialist clinical expertise, consultancy and advice providing clinical professional leadership within your area of expertise, using expert clinical knowledge and advanced practical expertise, and to be a resource for multi-professional team members and colleagues within the region, nationally and internationally.
  • To understand National Guidelines and legislation relating to health and social care in Occupational Therapy practice, recommending and implementing changes in service provision and practice accordingly and support staff in doing the same.
  • To identify and manage all risks associated with own clinical and professional practice and ensure all staff do likewise.
  • To be responsible for maintaining own and teams accurate, up to date and comprehensive client treatment records in line with College of Occupational Therapists (COT), HCPC and local standards of practice, and to ensure all staff are doing likewise within a specified time.
  • To possess and apply expert understanding of the effects of disability, disease and injury on occupational performance, lifestyle and independence. Provide specialist training, education and advice to clients and carers on managing and adapting their social and physical environments and assist patients/carers to implement effective psychological or cognitive coping strategies.
