Home Carer (Band 3)
The Domiciliary Care Worker [DCW] works as part of a Multi-Disciplinary Team to provide care & support to a wide range of individuals in their own homes. The individuals whom the DCW provides care and support to may have various needs including older people, people with disabilities, those with dementia, learning disabilities, mental health issues, palliative and end of life care. The DCW will provide care and support to these individuals as directed and in accordance with an agreed care & support plan. The Domiciliary Care Worker will discharge all designated duties in accordance with the relevant Trust policies and procedures and the Domiciliary Care Regulations and Minimum Standards.
Reablement Support Worker (Band 3)
The purpose of the Reablement service is to enable individuals to regain the skills and confidence they may have lost as a result of poor health or as a consequence of having spent a period of time in hospital or other care facility. The objective of the service is to enable individuals to live as independently as possible at home.
The Reablement Support Worker [RSW] is a key member of the Trust’s Reablement Service and will work under the guidance of Reablement Coordinator, Occupational Therapist and Team Leader.
The RSW works as part of a multi-disciplinary team providing personal, domestic, and social care and support at home to individuals (including frail elderly, physically disabled, and learning disabled, etc) and to their carers. The RSW will carry out prescribed tasks as delegated, monitored and reviewed by the Occupational Therapist, Reablement Coordinator and Team Lead to help the individual to achieve identified goals and objectives to maximise their independence.
The various tasks that the RSW may assist the individual with will include all or some of the following according to the individual goals and objectives as set by the Occupational Therapist and Reablement Coordinator and as set out in the individual’s Goal Plan. The RSW will discharge all designated duties in accordance with the relevant Trust policies and procedures and the Domiciliary Care Regulations and Minimum Standards.
