Location: Southern Sector of WHSCT in Omagh Hospital, South West Acute Hospital, Enniskillen, and outreaching to the community (base to be determined on appointment)

Contract and Hours: Permanent, Part Time (21.5hpw, 5.3PA’s across Monday to Friday)

Job summary:

Palliative Medicine was recognised as a specialty in 1987 and higher medical training programmes were established. This coincided with the rapid development of Palliative Care services, which included new links between community, hospice and hospital care and between the NHS and voluntary sector.

This is a permanent appointment. The post holder will join the existing Western Health and Social Care Trust (Western Trust) Specialist Palliative Care Team to primarily meet the Specialist Palliative Care needs of cancer and non-cancer patients within the Southern Sector of the Western Trust.

In this job description and specification the Southern Sector of the Western Trust refers to Omagh and Fermanagh areas.

Through an agreed Job Plan, the post holder will provide clinical sessions within the Southern Sector of the Western Trust. The post holder will do so in partnership with the established Specialist Palliative Medicine Team.

The post holder will contribute to the development and management of the Western Trust Specialist Palliative Care Services in a way that meets the need to deliver a service which is patient centred and holistic.

 

Main responsibilities

  • To be responsible for the assessment and management of patients referred to Specialist Palliative Care, providing ward visits, outpatient clinics and domiciliary visits.
  • To provide a consultative medical advisory service to other professionals within the Western Trust and in Primary Care.
  • To be clinically responsible for the 9 bedded Palliative Care ward in Omagh Hospital and Primary Care Complex (OHPCC).
  • To develop and participate in the multi-professional approach to patient care. To include attendance at relevant multi-disciplinary meetings, including the Western Trust weekly Palliative Care multi-disciplinary meeting.
  • To participate in the development of Palliative services particularly within community settings in line with strategic and Western Trust guidelines and policy. This will involve engagement through the Western Trust Palliative Locality Board, relevant subgroups and regional structures (e.g. Regional Palliative Medicine Group).
  • There will be a commitment to provide telephone cover in the first instance for Consultant colleagues from across the Trust during absence periods which will be reviewed as the team develops to work towards full cover. This commitment may change as the Team increases to its planned full complement of consultant staff.
  • The commitment to an on-call rota has not as yet been fully developed. This may become a component of the future substantive post-holders job plan. Job planning process will be adhered through engagement with the post holder in line with Western Trust policies.
  • There will be a commitment to fully participate in the training and teaching of the Integrated Care Fellows working in the Palliative Care department providing regular work based assessments, feedback and ward based teaching.
