Location: Lakeview Hospital, Gransha Park, Londonderry
Contract and hours: Temporary, Full Time (37.5hpw) until 18 July 2025 initially

Job purpose
The post holder will provide specialist Social Work support and function as a full member of the multi-disciplinary Forensic Community Adult Learning Disability Team providing services primarily working across the Limavady, Derry and Strabane areas. The postholder will be required to work closely with Southern Sector counterpart to ensure service delivery across the Trust area. He/she will be responsible for providing consultation, advice and shared working with other relevant agencies. There will also be responsibility for management of a small caseload.
Main responsibilities
- Assess care needs of clients, plan, implement and evaluate programmes of care.
- Develop care plans in conjunction with other members of the multi-disciplinary team and review as appropriate.
- Complete a comprehensive assessment of all risk factors inherent to each individual and develop an appropriate risk management plan.
- Develop client contact and monitoring in the Community to reduce admission rates and facilitate discharge.
- Ensure care plans are regularly updated and reflect the changing needs of the patient.
- Actively participate in the clinical supervision process.
- Research and actively seek out accommodation / housing for clients with forensic and offending behaviour issues in consultation with managers.
- Promote and maintain effective channels of communication within the service structure and with other agencies to facilitate multi-disciplinary co-operation.
- Support the development of day services with forensic needs through engagement in social activities and employment and education opportunities.
- Maintain clear lines of communication between Regional Secure Unit and Community Forensic Services.
- Develop risk assessment and action plan strategies.
- Attend Case Reviews and discharge planning meetings at Regional Secure Unit when required.
- Manage cases where clients are subject to Supervision and Treatment Order.
- Liaise as appropriate with Criminal Justice Services including PSNI, Probation and Courts.
- Establish links and care pathways with other agencies for the delivery of safe effective services i.e. Alcohol and Drug Treatment Unit, Probation Services, Community Mental Health Teams, Voluntary Services, Advocacy Groups, etc.
