Job summary

Reablement is a short term care and support service provided to people in their own home. The service is designed to help people gain or regain the necessary skills and confidence to live as independently as possible in their own home.

The post holder will manage the day-to-day provision of reablement services, ensuring the most effective service is provided to meet the assessed needs of the client taking account of available resources.

Services must be provided and managed to meet the NISCC Standards of Conduct and Practice and RQIA Domiciliary Care Minimum Standards.

Main responsibilities

  • To hold a caseload of reablement service users and be responsible for the facilitation, implementation, monitoring and review of these individuals receiving the reablement service.
  • Screening and assessment of Service Users needs in accordance with reablement protocol.
  • Establish case priority and as required undertake the evaluation of complex factors. Such judgements may be undertaken in unanticipated or emergency situations.
  • To work as part of a multi-disciplinary team and in collaboration with Acute and Primary and Community Care colleagues to assess Service Users referred to the reablement service.
  • To promote rehabilitation and reablement ethos enabling patients to achieve their full potential in terms of independent living, taking account of their health and social care needs.
  • To promote and facilitate the timely discharge of people from hospital to community settings.
  • Assess Service Users needs to help them to meet their reablement goals.
  • Implement administration of medication arrangements for Service Users where appropriate, in accordance with WHSCT guidelines.
  • Assess and implement modifications to Service User’s care plan in accordance with their reablement needs.
  • Work in partnership with patients, families and colleagues within Primary Community Care and Acute Services to identify and analyse risk associated with reablement care plan to facilitate safe delivery of services.
