Job summary

The Homecare Co-ordinator is responsible for the day-to-day management of in-house Homecare Care Services within a designated geographical area, ensuring the most effective service is provided to meet the assessed needs of the client taking account of available resources.

Services must be provided and managed to meet the NISCC Standards of Conduct and Practice and RQIA Domiciliary Care Regulations underpinned by Minimum Standards.

Main responsibilities

To manage the day-to-day provision of in-house Homecare services, ensuring the most effective service is provided to meet the assessed needs of the client taking account of the available resources. The postholder will ensure quality service provision, this will involve establishing case priority and as required; the evaluation of complex factors. Such judgements may be undertaken in unanticipated or emergency situations.
To co-ordinate arrangements for cover to clients to account for staff absences [sick leave, annual leave, special leave, etc], to advise and take action as appropriate.
To monitor the service provided, taking the appropriate action to ensure that standards of performance and quality of service are maintained.
Implement agreed service changes as directed by and in consultation with the key worker.
Participate in review meetings and case discussions to advise multi-disciplinary staff on relevant aspects of the Homecare provision particular to the case in question, e.g. availability of carers, complexity of provision, quality of standards, health & safety issues, risk, client and carer relationships, etc.
To organize and participate in regular staff meetings for the purpose of discussing issues and concerns regarding day to day operations and service developments.
Liaise with professionals from all programmes of care and with clients, their family and advocates to provide information, guidance and support on all aspects of Homecare service provision.
Participate in the risk assessment procedures in accordance with Trust standards and protocol.
Support and participate in projects and innovations to develop services.
To participate in the on-call service.
