Job summary
Team Leaders are part of the care team whose function is to meet the practical, physical and emotional needs of adults. Through a key worker system, he/she will also be responsible for designing individual programmes of care and co-ordinating the delivery of the package by a group of Support Workers and him/herself.
The residents within the home will all have a learning disability and may have challenging behaviour or profound care needs.
Main responsibilities

To ensure that all work within the residential unit is participant focused and upholds the principles of respect, privacy, dignity, fulfillment, independence and choice.

To plan and implement individual and group programmes of care, for example, social and domestic training, personal hygiene and community living skills designed to enhance and maximize the capabilities and independence of the residents of the unit.

To participate in monitoring and reviewing individual residents.

To establish goals on an individual and group basis so as to ensure the effective utilization of resources by the residents i.e. day care, home community, voluntary and statutory agencies.

To participate in meeting the physical and personal needs of residents. This may include toileting, feeding and bathing.

To ensure that menus are planned with residents in the home so that they receive nutritionally balanced meals, respecting the dignity and dietary requirements of preferences of each resident.

To ensure that medication is held, stored and administered in accordance with the Trust’s Medication Policy.

To intervene appropriately to resolve situations arising out of challenging behaviours/mental health difficulties. This may involve an element of physical risk to the employee. Staff will be fully trained in how to deal with these situations.

To provide effective leadership to Support Workers.

To co-ordinate the Support Worker’s input into individual programmes of care.
