We're looking for a contract Registered Nurse - Post Court Transition to join our Mental Health and Justice Services team!  

As part of the MH&JS team, the RN Case Manager will support individuals identified with complex health and mental health needs who will benefit from short-term transitional case management; facilitating successful transitions to the community following the end of their court matters. 

 

Status: 6 Month Contract until August 2025, full-time - 37.5 hours per week. Work hours will generally fall between 8:30am and 7:00pm. Note, this posting is for an active vacancy.

 

Location: This position is based out of our 314-7700 Hurontario office. Although we operate within a hybrid workplace model, staff are required to have on-site and/or community presence as per program and client needs. 

  

  

What you will do 

  

  • Provide community mental health case management with a focus on provide comprehensive nursing care and education as part of the Mental Health and Justice Services program 

  • Coordinate the involvement of the multidisciplinary team (to assess and to review previous assessments). 

  • Conduct medical, functional and educational assessments  

  • Strengthen the collaboration between service providers and primary care to improve case coordination  

  • Build/maintain partnerships with other service providers and referral sources.  

  • Maintain support systems and allow bridging to other services post court. 

  • Develop a comprehensive care plan supporting both mental health and health needs.  

  • Interview caregivers/families individuals involved in the individual’s life, gather prior assessments and records. 

  • Make appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment. 

  • Ensure that follow-up recommendations are appropriately coordinated and communicated 

  • Identify and maintain a list of resources and community supports  

  • Identify and address gaps in service and develop a proactive approach  

  • Develop and implement resources for primary care providers and others to help translate knowledge into practice 

  • Maintain accurate and up to date records, data management as required 

  • Developing and implementing quality improvement initiatives in engaging primary care, working with community partners and service provision 

 

What you need to join our team   

  • Bachelors Degree in Nursing (BScN) and current registration as a Registered Nurse with the College of Nurses of Ontario. 

  • A minimum of 4-5 years’ experience with individuals with mental health illness, both inpatient and outpatient in the community, preferred. 

  • Experience working with people with mental health and addictions who have experienced conflict with the law and exposure to the criminal justice system, is an asset 

  • Excellent assessment, crisis intervention and problem-solving skills. 

  • Excellent communication and interpersonal skillsDemonstrated ability to establish therapeutic, supportive relationships with client/family/caregivers/community agencies by utilizing skilled interviewing and communication techniques and appropriate nursing interventions. 

  • Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence. 

  • Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework. 

  • Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc. 

  • Fluency in a second language, including ASL (American Sign Language) is considered an asset. 

  • Valid Canadian driver’s license and access to an insured automobile in good repair 

  • A satisfactory Vulnerable Sector Screening (Police Check) report.
