Job summary
The post holder will lead, develop and deliver a high quality patient focused clinical pharmacy service in accordance with local and national priorities for patients within but not limited to Rehabilitation inpatient care Omagh hospital and primary care complex. He/she will work with patients, nursing, medical staff and other healthcare professionals to optimise medicines management on a patient and wider department basis. The postholder will work collaboratively with professional teams in primary and secondary care to provide high quality medicines management for patients using these services. The postholder will take an active role in training pharmacy and clinical staff on medicines management processes related to this clinical area.
Main responsibilities
- Lead, develop and deliver a comprehensive, specialist patient-focused clinical pharmacy service to patients within but not limited to the inpatient rehabilitation unit OHPCC in line with national/ regional guidelines and the regional medicines optimisation framework.
- Be an integral member of the rehabilitation teams and participate in multidisciplinary ward rounds and multidisciplinary meetings.
- Demonstrate the ability to provide safe, clinically effective and cost-effective use of medicines in accordance with Trust policy, standard operating procedures and medicine legislation. This will include medicines reconciliation on admission and discharge, autonomous prescribing and prescription review, advising on dosage and side effects and on safe prescribing and administration of medicines. The post holder is expected to determine how outcomes are best achieved in specific circumstances.
- Monitor key medicines management outcomes which align with the overall aims of the service.
- Ensure prudent antimicrobial prescribing by working collaboratively with antimicrobial stewardship teams and medical and nursing staff.
- Link closely with professional colleagues from transferring hospitals to ensure smooth transition to the rehabilitation unit.
- Provide expert advice and support to medical, nursing and pharmacy staff in primary and secondary care to ensure the safe and effective delivery of drug therapy to patients, including the off-licence use of drugs.
- Communicate specialist pharmaceutical care needs across care settings for patients with complex and multiple pharmaceutical care needs.
- Develop prescribing guidelines, protocols, procedures and clinical management plans for this clinical area in collaboration with appropriate stakeholders.
- Prescribe for patients in accordance with the Trust and Royal Pharmaceutical Society Prescribing Framework, relevant legislation and professional and/or policy.
- Support patient flow and oversee supply of medicines processes enhancing timely and safe administration. Support patients and carers to use medicines effectively.
- To act as a role model/leader for more junior pharmacists, participating in clinician ward rounds, multi-disciplinary team (MDT) meetings and morbidity and mortality review within rehabilitation.
- Be responsible for the co-ordination of training for pharmacists and other healthcare professionals on aspects of drug use within the assigned clinical areas.
- Where appropriate be an expert member of Network and Trust committees relating to the use of medicines with the aim of providing expert advice within rehabilitation.
- Assist with the implementation of regional and national guidelines and the feedback to the Trust on compliance with these guidelines.
- Promote the managed entry of new drugs.
- The post holder will undertake and publish practice research and enable team members to do likewise and identify areas for quality improvement and clinical audit.
- Develop the role of the specialist pharmacist through links with national bodies e.g. UKCPA.
- Ensure that work practices and the working environment conform to the requirements of current legislation and professional recommendations.
- Ensure the maintenance of appropriate documentation and records.
